Code of Conduct Violation Reporting Form

If you witnessed a player, coach or spectator violate the code of conduct, please submit a report by filling out the form below.

Code of Conduct Violations are managed by the City League Advisory Council. The purpose of the Advisory Council is to support the Commissioner to make decisions that reflect the values and mission of the league. After filling out this form to report an infraction, a City League staff member will follow up with you within 5 business days.
