Officials, please complete the following form for each contest you officiate. The information will be submitted to the City League Commissioner, Allison Cousins.

Your payment will be mailed to you within 10 business days of the completion of this form. You must have completed the Officials Onboarding Form prior to receiving payment. A link to the onboarding form can be found in the League Handbook at each contest or can be distributed through Reed Hager or allison.cousins@stlcityleague.com.
